Back to Work Physio After a Work Injury in NSW

People searching for back to work physical therapy after a workplace injury are usually asking two things at once. Will my shoulder, back or knee handle the job again, and what happens to my claim while I build back up? Rehabilitation on a NSW workers compensation claim runs alongside the paperwork, and the two need to move together.

This is the part of a work injury claim where progress often stalls. The acute pain settles, the certificate still says partial capacity, and nobody has mapped out how you get from lifting 5kg at waist height to a full shift on the tools.

What return-to-work rehab is aiming at

The target is the job, described in detail. Not "labouring" but repeated 20kg lifts from ground to shoulder, 40 to 50 times across a shift, on uneven ground. Not "office work" but six hours of seated screen time with a 45-minute drive at each end.

So the first job in the clinic is to write down what your work actually demands. Loads, heights, reach, repetition, shift length, rest breaks, driving, ladders, night work, and whether your employer has lighter tasks available. Physiotherapy then works backwards from that list. A rehab program built around a real task list looks different from a generic strengthening program, and it gives your employer and insurer something concrete to approve.

The same logic applies to sport. Plenty of our patients are club cricketers, touch players and weekend runners who also work physically demanding jobs. Getting back to a Saturday game and getting back to a Monday shift draw on the same capacity, and both belong in the plan.

How the paperwork fits around treatment

Your nominated treating doctor issues a NSW certificate of capacity. That document records whether you have no current capacity, partial capacity with restrictions, or full capacity, and it is what the insurer and employer work from.

If the certificate says partial capacity, your employer is expected to look at suitable work within those restrictions. That might be reduced hours, a different task mix, or a temporary role. A recovery at work plan sets out the duties, the hours and the review dates in writing.

Physiotherapy feeds this process. We can report on what you tolerated in the clinic, where the current restrictions sit, and what the next step up might reasonably look like. Under SIRA's allied health guidelines an initial block of treatment can generally begin without pre-approval, with further sessions requested from the insurer in writing using a recovery request that includes goals and a review date. Keeping those goals tied to job tasks tends to make the conversation with the insurer clearer.

What assessment involves

A return-to-work assessment at our Five Dock clinic usually runs 45 to 60 minutes and covers:

  • The injury history, current symptoms, and what has already been tried
  • Movement testing and strength testing of the injured area and the regions around it
  • Loaded testing where appropriate, such as lifting from different heights, carrying, squatting, sustained postures or repeated reaching
  • Your certificate of capacity and current restrictions
  • A task-by-task review of your job and the duties currently available

From there we set a starting load, a weekly progression, and a plan for reassessment. We will also flag what we think needs medical review rather than physiotherapy.

Building capacity in stages

Graded loading is the common approach in work injury rehabilitation. You start at a level you can manage with acceptable symptoms, then increase load, reps or duration in steps, with the response to each step guiding the next one.

For a back injury that might begin with hip hinge patterning and trunk endurance work, move to kettlebell deadlifts and farmer's carries, then into ground-to-shoulder lifts at the weights your job demands. For a rotator cuff problem it might run from isometric holds and band work through to overhead pressing and sustained overhead tasks. For a knee it usually means squat and step-down strength, then change of direction and impact tolerance if the job or sport needs it.

Hours matter as much as kilos. Someone cleared for four hours a day may cope fine with the load but not the accumulated standing. Building shift duration deserves its own progression.

Exercise physiology and longer claims

On claims running several months, accredited exercise physiology often carries the heavier conditioning work. Sessions can be based in a gym, use work-specific equipment, and run longer than a standard physiotherapy appointment. Both services can be funded on a workers compensation claim with insurer approval, and they are frequently used in sequence: physiotherapy early for the injury itself, exercise physiology for the capacity build once symptoms allow.

Common sticking points

Fear of re-injury is a normal response, particularly after an injury that happened suddenly at work. Testing capacity in a controlled setting, with loads you can see on the bar, gives you information rather than guesswork.

Other things that hold claims up include duties that exist on paper but not on site, a certificate that has not been updated after a clear change in capacity, and a program that never progresses past low-level exercise. If any of those apply, raise it at your next appointment so it can be addressed in the plan and the reporting.

When to seek help

Early contact is generally easier than late contact. If you have a current certificate of capacity, a claim number and restrictions you are working around, that is enough to start. It is also worth booking if a return to work has already been attempted and did not hold.
